These radioactive drugs specifically target cancer cells by combining a molecule capable of recognizing tumors with a radioactive element. This approach, known as <em>theranostics<\/em>, allows both the diagnosis and treatment of cancer using the same targeting mechanism. It relies on medical imaging to identify eligible patients and tailor treatment based on the tumor&#8217;s response.<\/p>\n<p>Recent advances focus on treating prostate cancer, particularly through a protein called PSMA, present on the surface of cancer cells. Drugs such as lutetium-177-PSMA-617 have demonstrated their effectiveness in extending the survival of patients with advanced forms of this disease. Clinical trials show that this therapy significantly improves progression-free survival and reduces the risk of death. It is now used before chemotherapy for some patients, offering a valuable alternative.<\/p>\n<p>PET-scan imaging, which visualizes the presence of PSMA, plays a key role in patient selection and treatment monitoring. Specific criteria have been developed to assess treatment response, thereby avoiding errors associated with traditional methods.<\/p>\n<p>However, some patients do not respond to these therapies due to resistance mechanisms such as the presence of PSMA-free cells or insufficient radiation distribution. To overcome these obstacles, research is exploring the use of more powerful and targeted alpha particles or combining them with other treatments such as immunotherapy.<\/p>\n<p>Beyond prostate cancer, this approach is extending to other types of tumors, such as breast cancer. Specific molecular targets, such as HER2 or FAP, are being studied to develop tailored radiopharmaceuticals. These innovations could transform the management of many cancers, provided that healthcare systems adapt to ensure equitable access to these treatments.<\/p>\n<p>The future of radiopharmaceutical therapies will depend on the ability to train professionals, optimize the production of radioactive drugs, and integrate these innovations into care protocols.
